Specific feedback or suggestions

The Friends and Family Survey above is designed to give us a quick snapshot of the general service the practice provides. Your answers to the survey are completely anonymous. From time to time, you might want to send us some feedback, ask a question or make a suggestion on a more specific subject. We welcome your comments but we need to collect your name and email address in order to be able to reply. We will use these purely for the purpose of replying and your details will not be stored anywhere.

Neither should it be used to communicate confidential informtion about you or anyone else. if the content of your message is of a personal or confidential nature, please call in to the practice or telephone and ask to speak to a member of staff.
