Register online

Use this service to register with a GP surgery for the first time or to change your current GP surgery. The registration process will take around 15 minutes to complete. You will need to complete the process in one session. Your chosen GP surgery should register you within 2 weeks of receiving your details.

New patient registration

All UK residents have the right within the NHS to be registered with a doctor. We are pleased to register anyone who lives within the practice area.

Please visit the surgery and talk to our receptionists. It would be a great help if you could bring with you your up-to-date medical card. If you cannot find it, do not despair. Simply see our receptionist and ensure you know the following details:

  • Full name, including previous names
  • Date and place of birth
  • Last address
  • New address and post code
  • Last name and address of your previous Doctor
  • NHS number - if not known (it is not the same as your National Insurance number) enquire from your previous doctor before attending to register.

Please let the receptionist know when you come in to register with the practice if

  • you need information in braille, large print or easy read
  • you need a British sign language interpreter or advocate
  • we can support you to lip read of use a hearing aid or communication tool

Please note you must provide the name of your previous doctor. If you do not know, or cannot remember, please contact your previous surgery for the necessary information. We need this information to be able to register you with our practice.

Registration Forms

You will need to complete an NHS (GMS1) Registration Form for each member of your household when you register in person. This is only available from the Surgery.

As it takes a while for your medical records to transfer to our practice we also ask you to also fill in our 'personal history' form for each person being registered.

You will need to print a separate form for each member of your household who wishes to register as a patient with us.

New patient health check

Every patient registering with the practice will have the opportunity of a new patient consultation. The appointment gives you a chance to meet us, have your health checked and bring to light any ongoing medical problems or important past medical history, including continuing your medication.

Your appointment may be with a practice nurse.

We will ask you about the following:

  • Your past medical history (important illnesses and operations)
  • Ongoing medical problems
  • Medication
  • Family history
  • You will have your blood pressure taken and your urine will be tested. Occasionally other examinations or tests may be done if needed.

Accountable GP

As part of the NHS commitment to providing more personalised care, from June 2015 all practices are required to provide all their Patients with an accountable GP who will have overall responsibility for the care and support that our surgery provides. This will not impact your experience at the practice, the provision of appointments, your treatment, or which GP you can see.

You may wonder why your allocated GP is not necessarily the one you see most regularly. Please be assured that you can still access all of our medical team in exactly the same way as before. Having a named GP does not guarantee you will always be seen by that GP. Please note that the GP responsible for your care may be subject to change and reallocation in the future.
