We would like your feedback on the care or treatment we give you any time you visit your GP or have contact with the practice. Please take a few minutes to complete our online questionnaire.

Put us to the test and tell us what is working and what we can improve. You can say what you think without giving your name, and we will use the information to plan improvements to our services.

Specific feedback or suggestions

The Friends and Family survey above gives us a quick snapshot of the general service the practice provides, and your answers to it are completely anonymous. From time to time you might want to send us feedback, ask a question or make a suggestion on a more specific subject. We welcome your comments, but we need your name and email address so that we can reply. We will use these purely to reply to you, and your details will not be stored anywhere.

Please note: this form is checked every day but is not monitored continuously. Do not use it to send medical requests, urgent or otherwise, and do not use it to send confidential information about yourself or anyone else. If your message is personal or confidential, please call in to the practice or telephone and ask to speak to a member of staff.

This form should not be used to make a formal complaint. Please use our complaints procedure for that.
